RAID
One RAID log, with every entry linked to the work it affects.
Risks carry probability, impact and a score, a mitigation and a contingency, an owner and a target date. The heatmap shows where the exposure sits, and a risk that happens becomes an issue in one step.
Issues record severity, blockers, root cause and workaround. Actions and decisions keep the meeting they came from, so the reason for a decision does not live in someone's inbox.

RAID
Risks, issues, actions and decisions in linked registers.
What the module covers
Scored risks and a heatmap
Probability times impact gives the score. Filter by category and status, or click a heatmap cell to see the risks behind it.
Issues that show what is blocked
Severity, priority, a blocker flag, environment, root cause, workaround and resolution, linked to the task and the risk they came from.
Actions with a source
Each action has an owner, a due date and the meeting that raised it. Close several at once when the work is done.
A decision record
Design decisions with their rationale, the alternatives considered, the impact, who decided, and which later decision replaced them.
In the app, this module is these workspace tabs:
- Risks
- Issues
- Actions
- Decisions
How a programme uses it
Raise it where it belongs
Log a risk, issue, action or decision with an owner and a date, linked to the task or meeting it relates to.
Review it on a rhythm
Use the heatmap and the blocker filter in the weekly review, and agree the next step for each item.
Close the loop
Convert risks that happen into issues, mark actions complete, and supersede decisions that change.
Where it helps most
Client-side PMOs
Run the weekly RAID review from one log, with each item linked to the task or meeting it belongs to.

Give the client the same risk and issue view your team works from, so escalations start from shared facts.

RAID: common questions
How is the risk score calculated?
Probability multiplied by impact. When a risk is converted to an issue, the issue priority is set from the score.
Can actions come from meetings?
Yes. Actions and decisions record the meeting that raised them, and each meeting lists the actions and decisions linked to it.
